What We Do Best
When you choose FAHI Home-Based Care, you’re choosing a provider that focuses on the three things that matter most:
1) Safety First
We reduce risk at home through:
- fall prevention and safe mobility support
- medication adherence support and monitoring
- wound and infection prevention
- close observation and timely escalation when a client deteriorates
2) Dignity & Comfort
We protect dignity through:
- respectful personal care (ADLs)
- privacy, professionalism, and compassionate communication
- consistent routines that support independence
3) Family Peace of Mind
We support families with:
- reliable caregivers with clinical oversight
- flexible scheduling and continuity of care
- clear updates and accountable service delivery
Patient Assessment Comes First
Before we start care (and during care when needed), we conduct a professional patient assessment to ensure the client is safe and the care plan is accurate. This helps us:
- understand the client’s medical condition and daily needs.
- identify risks (falls, pressure sores, medication errors, dehydration, infections)
- determine the right level of care (caregiver vs nurse vs clinician support)
- plan equipment needs (mobility aids, oxygen support, CPAP/BiPAP support)
- set clear goals and routines for recovery, comfort, and independence
